National Survey on Advance Care Planning – Please Promote and Complete It! 

The National Healthcare Decisions Day (NHDD) initiative is conducting a national survey to assess public knowledge and attitudes about advance care planning – and we would like your organization to participate.  The survey questions are based on previous surveys of advance care planning and end-of-life issues and the goal of this survey is to gain a more current assessment of the issues surrounding advance care planning issues.  Key results from the survey will be posted on the NHDD Web site and included in press materials accessible to all NHDD participants.

The survey can be completed in 5 minutes or less and the deadline for completion is March 24, 2008.

We ask that you please help us with this survey by:

1. Completing the survey yourself; 
2. Forwarding the survey information and link to people working within your organization; and 
3. Forwarding the survey information and link to members or constituents associated with your organization.
